Our History

The Perry County Health Department's Home Health Agency has provided home health services since 1977. The agency serves residents of Perry County and surrounding areas with Skilled Nursing Care, Home Health Aide Services, Nutrition management, Physical Therapy, Speech, and Medical Social Services in the home when prescribed by the patient's physician.

Persons qualifying for care in the home include the recently discharged hospital paitent, chronically ill or disabled, the recuperating and terminally ill patients who are homebound and require a skilled nursing service on an intermittent, temporary basis.

Our goal is to assist each person to reach the highest possible level of independence and to assist each person to remain with loved ones in their own homes with the highest quality of life. Clients, family members and significant others are encouraged to actively assist in the client's plan of care and recovery. Clients are accepted for home care regardless of financial status or payment source.
